The West Sussex CHC Team are working from home during the pandemic with no physical patient or colleague contact. We have worked with our health and social care partners to ensure all can be done to support hospital discharges during this unprecedented demand for NHS acute services.

Please note: the West Sussex Continuing Healthcare and Children and Young People’s Continuing Care team previously worked on behalf of Coastal, Crawley, Horsham and Mid-Sussex Clinical Commissioning Groups (CCGs) and following changes on 1 April 2020 the team now represent the new West Sussex CCG. NHS Continuing Healthcare (CHC) is the name given to a package of care that is arranged and funded solely by the NHS for individuals who are not in hospital but have complex on-going healthcare needs.
